Since 1985, Progressive has provided Home Health Aide and Personal Care Aide services through contracts with most of the Certified Home Health Agencies (CHHAs) and Long Term Home Health Care Programs (LTHHCPs) in New York City. Our mission is to provide cost effective Home Health Services of the highest quality. Progressive Home Health Services contracts with various New York certified Home Health Aide agencies to provide quality home care services.Progressive has received the highest service ratings from these agencies. As a result, Progressive has distinguished itself by obtaining contracts to provide many specialty programs. In 1999, Progressive was awarded a contract with the New York City Human Resources Administration and began providing home attendant services in 2001 doing business as Kind Care. In 2010, Progressive received Accreditation with Commendation from the Joint Commission on Accreditation of Healthcare Organizations. We also had other audits from many of our contracts in which we received outstanding scores on all. We also offer services now through our Private Pay Division. Progressive has invested in technologies to support its management vision. We have developed and continue to expand capabilities in management systems and cutting edge technologies necessary to improve quality of care. The potential of this mission-driven agency is best represented in the growth and quality of the services provided to its patients. We service many different types of clients- the elderly, disabled, anyone seeking to manage independent care within their home. Our staff has a variety of languages available to meet client needs: Spanish, Chinese (three dialects), Haitian Creole, and Russian.
Patient Rights
Bill of Patient Rights and Responsibilities
Patients have the right to be informed of these rights, in writing, in a way that the patient understands, prior to the initiation of care and the right to exercise such rights. Practice/Procedure/Implementation:As a patient you have the right to:1. Competent, concerned, individualized care without regard to race, creed, color, age, sex, national origins, religion, sexual preference or disability.2. Be treated with dignity, consideration, and respect, including respect for your privacy, your property, your safety/security and your personal cultural and ethnic, psychosocial, spiritual, personal values, beliefs, and preferences. You have the right to complain of disrespect for your property.3. Be free from real or perceived mental, physical, sexual, and verbal abuse, neglect, and exploitation; and to have all allegations, observations, or suspected abuse, neglect or exploitation investigated.4. Expect the agency to maintain a written care plan, and to include your participation, as able, in all decisions affecting your care and treatment. This includes an emergency disaster plan. Be advised in advance of any changes in your Plan of Care.5. Privacy and confidentiality of all information, including records pertaining to your treatment, except as otherwise provided by law or third-party payment.6. Access, request amendment to, and receive an accounting of disclosures regarding your health information as well as to request restrictions of use and disclosure and receive confidential communication.7. Know the names and functions of those people responsible for coordinating, rendering and supervising your health care, including the identity of other health care providers with which the agency has contractual relationships, and to expect that staff members will listen to voiced concerns and/or complaints. Be informed of all services the agency will provide, when and how such services will be provided.8. Be fully informed of your diagnosis, prognosis and treatment, including alternatives to care and risks involved, with explanation by staff who are knowledgeable of your condition. Information will be presented in a form, which you can understand.9. Make informed decisions about proposed and ongoing care or services. Refuse treatment after being fully informed of and understanding the consequences of such actions, without fear of discrimination or reprisal.10. Receive information regarding the agency's policies and procedures, including the discontinuation of service, and to be informed of continuing health care needs following transfer or discharge and to be involved in the plan for the provision of such care.11. Have your complaints heard, reviewed, and if possible, resolved and recommend changes in policies and services to the agency staff, the governing board, the New York State Department of Health (212-417-5888) and JCAHO without fear of reprisal, coercion, interference, discrimination, or unreasonable interruption in service. To file a grievance or to receive information in writing on how to file a grievance, write the Administrator of the program at 132 West 31st Street, 7th Floor, New York, NY 10001. This address may also be used to submit complaints about the care and services provided or not provided, and complaints concerning lack of respect and lack of respect of property.12. View your clinical record in the agency office or receive a copy of it.13. Examine, question and receive a full explanation of any bill regardless of source of payment.14. All rights and responsibilities specified as they pertain to a patient lacking capacity to exercise these rights, will be exercised by an individual, guardian or entity legally authorized to represent the patient.15. Be given a statement of services available by the agency and related charges, including the actual dollar amount to be borne by you; be advised prior to initiation of care of the extent to which payment for agency services may be expected from any third party payor and the extent from which payment may be required from the patient, to be advised of any changes in such as soon as possible but no later than 30 calendar days from the date agency was aware of the change. This information shall be provided in writing and orally.16. Be given information on patient self-determination, Advance Directives, health care proxy, living will and DNR. Existence or lack of Advance Directives does not hamper your access to care or services.17. Be given information in a language or form that you can reasonably understand.18. Information about the ownership of the agency.19. Not to receive experimental treatment or participate in research unless you have given documented voluntary informed consent.20. Participate in ethical issues and the resolution of conflicts arising in your care.21. Be referred to other organizations, services, or individuals and be informed of any financial benefit to the referring home care organization.22. Appropriate assessment of your reports of pain by concerned, qualified staff, as well as information about pain, pain relief measures and the effective management of your pain.THE PATIENT'S FAMILY OR GUARDIAN MAY EXERCISE THE PATIENT'S RIGHTS WHEN THE PATIENT HAS BEEN JUDGED INCOMPETENT.Patient Responsibilities (Including Caregivers and Family Members)The agency expects that the patient will:1. Be seen by your doctor if a change in your health status occurs.2. Share complete and accurate health information with agency staff, including present complaints, pain, past illnesses, hospitalizations, medications and your pain management regimen.3. Inform staff of any changes in your health status and make it known if you do not understand or cannot follow instructions. Ask questions about your care and/or service.4. Be responsible for following the treatment plan recommended by the agency and advise the agency of any problems or dissatisfaction with your services.5. Cooperate with and be respectful of the agencys personnel and property and not discriminate against staff because of race, creed, color, sex, age, sexual orientation, religion, national origin or handicap.6. Notify the agency in advance whenever you find you will not be home at the time of your next scheduled agency visit.7. Notify the agency if you receive services from any other agency of facility and/or if you have rental equipment, which you no longer use.8. Be responsible for your actions if you refuse treatment or do not follow the agency's instructions.9. Be responsible for assuring that the financial obligations of your health care are fulfilled as promptly as possible.10. Notify the agency of the existence of and any changes made to Advance Directives.11. THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY. Normally, this documentation contains information regarding your health: diagnosis, symptoms, examination, treatment provided, test results, communications in the course of your treatment, and an ongoing plan for your care and treatment. This information, often referred to as your health, medical, or clinical record, serves as a basis for planning your care and treatment and serves as a means of communication among the many health professionals who participate in your care. Understanding what is in your health care record and how your health information is used helps you to ensure its accuracy, better understand who, what, when, where, and why others may access your health information, and will help you make more informed decisions when asked for authorization to disclose this information to others. YOUR RIGHTS REGARDING YOUR HEALTH INFORMATION:Unless otherwise required by law, your health care record is the physical property of the healthcare provider (hospital, physician, home care agency, or other) that compiled it, but the information belongs to you. You have the right to request a restriction on certain uses and disclosures of your information, and request amendments to your health care record. This includes the right to obtain a paper copy of the notice of information practices upon request, inspect, and obtain a copy of your health record. Obtain an accounting of disclosures of your health information, request communications of your health information by alternative means or at alternative locations, revoke your authorization to use or disclose health information except to the extent that action has already been taken. THE AGENCYS RESPONSIBILITIES:This agency is required to maintain the privacy of your health information. In addition, we are required to provide you with a notice as to our legal duties and privacy practices with respect to information we collect and maintain about you. The agency must abide by the terms of this notice, notify you if we are unable to agree to a requested restriction, accommodate reasonable requests you may have to communicate health information by alternative means or at alternative locations. We reserve the right to change our practices and to make the new provisions effective for all protected health information we maintain. Should our information practices change, we will hand deliver or mail a revised notice to the address youve supplied us. When we maintain a Web site that provides information about our customer services or benefits we will post our new notice on that Web site. We will not use or disclose your health information without your authorization, except as described in this notice. EXAMPLES OF DISCLOSURE FOR TREATMENT, PAYMENT, AND HEALTH OPERATIONS:The agency staff will utilize your health information for treatment. The health information you provide or obtained by the agency will be recorded in your clinical record. This information will be used to develop your individual plan of care. For example, your nurse will document the results of your initial assessment in your clinical record. From this assessment your plan of care will be developed with your input and orders from your physician. The plan of care will include what kind of aide services you require, how often, what this specific service is expected to do, and what outcomes are expected from aide service as well as your expected overall outcomes. Members of your health care team (i.e., nurse, aide) will document the care they provided and their observations. We will also provide each member of your health care team with the information they need in order to care for you. For example, members will be provided with information regarding your advance directives if you have chosen to execute an advance directive. The agency will utilize your health information for payment. For example, a bill will be sent to you or a third-party payer, or whoever is responsible to pay the bill for your care. The information on the bill may include information that identifies you, your diagnosis, the type and amount of service you received. The agency will utilize your health information for health operations. For example, members of the quality improvement team may review your record as well as other patient's records and utilize the information to assess the care and outcomes from the care. This information will be utilized in an effort to continually improve the quality and effectiveness of the care and services that the agency provides. There may be some services provided to our agency through contracts with Business Associates (BA). Examples include consultants we use to audit clinical records, policies and procedures and our business operations to ensure compliance with Federal, State and Local Regulations and quality standards. Other examples of Business Associated include individuals or organizations that may provide the following services: legal, actuarial, accounting, management, administrative, accreditation, data aggregation or financial services. When we utilize these services, we do so under contract with the individual or organization. We may disclose parts or all of your health information to our Business Associate so that they can do the task we asked them to do. In order to protect your health information, we include a clause in the contract about the need for the Business Associate to also safeguard your information. Notification: We may use or disclose information about you in order to notify a family member, caregiver, or a person responsible for your care, your location, and your general condition. Communication With Family: The agency or its professional staff, using their best judgment, may disclose to a family member, other relative, close personal friend or any other person you identify, your health information relevant to that persons involvement in your care or the payment related to your care. Research: We may disclose information to researchers when you choose to participate in a research project. Disclosure will be made only after review of the research protocols ensure that your health information will remain private. Funeral Directors: We may disclose your health information to a funeral director as required under law so that they can carry out their duties. Organ Procurement Organizations: Consistent with your wishes and with applicable law, we may disclose health information to organ procurement organizations or other entities engaged in the procurement, banking, or transplantation of organs for the purpose of tissue donation or transplant. Marketing: We may contact you to provide appointment reminders or information about treatment alternatives or other health-related services that may be of interest to you. Food and Drug Administration (FDA): As required by law, we may disclose your health information to the FDA if you should experience an adverse event with food, supplements, product and product defects, or information that will enable product recalls, repairs, or replacements. Workers Compensation: We may disclose your health information that is necessary to comply with laws related to workers compensation or other similar programs established by law. Correctional Institutions: If you are or become an inmate of a correctional institution, we may disclose your health information to the institution or a representative of the institution. This health information is necessary for your health as well as the health and safety of others. An inmate does not have the right to the Notice of Privacy Practices. Law Enforcement: As required by law, we may disclose health information for law enforcement purposes or in response to a valid subpoena. Federal law requires release of your health information to appropriate health oversight agencies, public health authority or attorney, if a staff member or Business Associate believes that we have engaged in unlawful conduct or have otherwise violated professional or clinical standards and are potentially endangering one or more patients, workers or the public. Other Health Care Providers: The agency may provide your protected health information to any hospital, nursing home, or other health care facility to which you have been admitted; to an assisted living or personal care facility of which you are a resident; to any physician providing you care; to licensing or state agencies acting as a representative of the Medicare/Medicaid programs. Emergency Treatment: The agency may use or disclose your protected health information for treatment in emergency situations. Public Health: We may disclose your protected health information, as required by law, to public health or legal authorities responsible for tracking births and deaths, as well as with preventing or controlling disease, injury, or disability. We may also use or disclose your protected health information as required by law, for health care oversight activities or to avert a serious threat to health and safety. Abuse, Neglect or Domestic Violence: If we reasonably believe you are a victim of abuse, neglect or domestic violence we will disclose your protected health information to the appropriate government authority. Law: As required by law, we may provide treatment, use or disclose health information as required by law or disclose for law enforcement purposes, in response to a valid subpoena, court order or with a protective order, for certain judicial administrative proceedings or certain law enforcement purposes. Federal law requires release of your health information to appropriate health oversight agencies, public health authority or attorney, or if a staff member or Business Associate believes that we have engaged in unlawful conduct or have otherwise violated professional or clinical standards and are potentially endangering one or more patients, workers or the public. Special government functions: This includes disclosure for military and veterans activities, national security and intelligence activities, protective services for the President and others, medical suitability determinations, correctional institutional and custodial situations. Agency Roster: Use of your name and demographic information to identify you as a patient of the agency. Notice of Privacy Practices: This hard copy of this privacy notice will be provided to you and all patients upon admission to the agency. If I need to get in touch with someone over the weekend, on a holiday, or during the evening, will someone be available to help me?Yes, Clinical and Administrative staff on call 24 hours a day, 7 days a week including holidays. Do you have a service that provides someone to escort me to and from doctor's appointments or to the hospital?Yes, we provide personal companion/ escort serivices to accompany all our clients to and from any predetermined destination.Do you provide senior services to those in nursing homes or assisted living facilities?Yes, we provide eldercare and companionship to seniors who may require additional attention and/or personalized long term care and reside in assisted living facilities and nursing homes.How are the aide's selected?First, Progressive looks for people whom have a passion for working with persons with disabilities to help them live independently. Then each potential care giver must successfully complete a thorough personal, professional, and criminal background check. How are the aide's trained?Progressive aide's participate in an exclusive care training program, which covers topics such as communicating with seniors, planning activities, recognizing illnesses, safety training, and a multitude of other issues. The aide's also benefit from regular team meetings and on-going training opportunities to enhance their skills.Who instructs the aide on what to do for me?A trained home care Registered Nurse will visit you to assess your needs. They will obtain doctors orders that outline what type of services you will need. The RN will make a plan of care especially for you. The aide will then be oriented to your plan of care by the RN at your home so they know exactly what to do to meet your specific needs. The RN will continue to supervise the care you are receiving based on your doctor's orders.What happens if my aide gets sick or can't make an appointment?Progressive provides 24/7 staffing services, so if one aide is unable to fulfill their duties, we make every effort to arrange for another team member to meet your home care needs.What is private pay? Private Pay was established for clients who are requesting services and are paying out of pocket. Clients sometimes do not quality for the benefits of home care services through their insurance and/or the government. With Private Pay it gives clients the opportunity to have home care services they need and want to be able to maintain their health and well being while in the comfort of their own home. What forms of pay are accepted?You can pay by Credit Card, Personal check, or Insurance.What is the difference between a "licensed agency" and a "certified agency"?A licensed agency, like Progressive is licensed by the New York State Department of Health to provide home care services, personal care and companionship, private duty nursing, escort services, and geriatric care services, among other services. A certified agency is certified to bill Medicare and/or Medicaid. This is something a licensed agency cannot do. Licensed agencies can provide many of the same services as a certified agency, so Progressive can provide the same services, but the referral for services doesnt need to come from your physician and the payment is made by the person receiving services, a guarantor (someone who accepts legal responsibility to pay for the services received on behalf of the patient), or through a long-term care insurance policy.How far in advance must I call to set up services?Depending on the type of service you require, you can recieve aide the same day. For instance our companion/ escort service can be supplied on demand. For other care service needs, it can be as short as 24 hours.

This service is designed to help those individuals who require assistance in some of their daily living activities and are otherwise healthy. Our companions help keep your loved one mentally healthy and alert by having conversations with them, accompanying them to religious functions and many other social/ secular activites.Our aides can provide homemaking services to maintani your loved one's independence. Homemaking services involve the tasks necessary for the upkeep and cleanliness of the home, including but not limited to:- Personal needs including assistance feeding, bathing, and dressing. - Meal planning and preparation including grocery shopping, monitoring diet and food intake, and clean-up. - Errands such as picking up prescriptions, gathering the mail, and general shopping.- Light housekeeping chores.

Private Pay Process
We have listed below the simple steps to begin private pay services here with us: 1 - Before one of our certified caregivers is selected to provide health care services, a Registered Nurse will visit the patient at his or home or in a facility. 2 - We will conduct a home care assessment to determine the patient's medical, physical, and personal needs. 3 - We learn about the patient's physical condition, abilities, dietary restrictions, and medications he or she is taking. 4 - We then carefully match the caregiver's capabilities with the specific needs of the patient. This home care assessment is used to develop a plan of care for the patient.5 - If the client has long term care health insurance, the plan of care will be necessary for reimbursement.For our patients who have long term care insurance policies, our Reimbursement Specialist assists in gathering and submitting all paperwork required by the insurance company for reimbursement of our services. We serve the five boroughs of New York and Westchester. We now accept most Long Term Care insurance plans, as well as personal check and credit cards for payment of services. We will verify your insurance coverage for you and will even take on the extra expense of filing your claim at no cost to you.

Welcome to Kind Care Home Attendant Services. Kind Care's mission is to provide our clients with personal care services that assist them in dealing with illness or disability, while maintaining their place in the community with independence and dignity.Progressive dba in Brooklyn provides Home Attendant services to Medicaid eligible individuals through a contract with the New York City Human Resources Administration (HRA) since 2002. Eligibility determinations and approval of services are determined by HRA.Home Attendant services include assistance with personal care (i.e. feeding, bathing, personal grooming, dressing, etc) and household tasks (i.e. cleaning, laundry, shopping, cooking, etc).Kind Care adheres to the New York State Department of Health and New York City Human Resources Administration regulations regarding home care services. At Kind Care, our clients receive quality services from the staff person caring for them, as well as a number of important support services, including:Clinical and Administrative staff on call 24 hours a day, 7 days a week including holidays.
